Our podiatrists and professional staff at Southeastern Foot & Ankle want you to have a successful experience at each of your visits. To help us serve you better, here are a few suggestions to help you prepare for an appointment.

What to do ahead of time:

  • Print out and complete the patient forms available on this website and follow the instructions provided.
  • Make a list of questions you’d like to ask the doctor.
  • Make a list of all medications you are currently taking, including any over-the-counter vitamins and supplements.

What to bring with you to your appointment:

  • Your completed forms, list of questions, and list of current medications;
  • A valid photo ID;
  • Your insurance card or other health coverage information;
  • Notes or operative reports from your referring physician, if available, that relate to your condition.

What to do during your visit:

Take an active role in your examination! Ask questions if you don’t understand terms used or instructions you are given.
